Safe Life Home Health Care, LLC is seeking a Home Health Registered Nurse (RN) to provide comprehensive nursing care in patients' homes across Melrose Park, IL. The role requires at least 1 year of home health experience and proficiency with Well Sky Kinnser EMR.

The RN will assess patients, develop individual care plans, administer medications, and collaborate with a multidisciplinary team to ensure safe, effective treatment and timely discharge planning.

Competenze

• Active Illinois RN license required.
• CPR certification required.
• Minimum 1 year home health experience.
• Proficiency with Well Sky Kinnser EMR.
• Valid driver’s license and auto insurance.
• Strong clinical assessment and communication skills.
• Compassionate and patient-centered approach.

Mansioni

• Provide skilled home health nursing care to patients in their homes.
• Assess health status, develop and implement care plans.
• Administer medications and treatments per physician orders.
• Monitor progress and document changes in condition.
• Educate patients and families on disease management and safety.
