Qualifications

Valid Illinois RN license.
Valid CPR Certification.
Valid Driver's License.
Valid Auto Insurance.
Minimum 1 year home health experience.
Proficiency in WellSky Kinnser EMR.
Strong clinical assessment and communication skills.
Compassionate and patient-centered approach.

Responsibilities

Provide strong clinical skills and patient-centered care with excellent communication.
Conduct comprehensive home health assessments and develop care plans.
Administer medications and treatments per physician orders with safety.
Monitor progress and document changes in condition and response to treatment.
Educate patients and families on disease processes, medications, pain management, fall prevention, and nutrition/exercise.
Collaborate with interdisciplinary teams for discharge planning and referrals.
Maintain accurate medical records in compliance with regulations.
Respond to inquiries and participate in staff meetings and conferences.
