Job summary

CenterWell Home Health is seeking a Home Health Registered Nurse Case Manager to provide personalized, in-home nursing care and collaborate with physicians and clinicians to plan and monitor patient recovery.

You will educate patients and families, coordinate care, and supervise Home Health Aides while ensuring documentation and reporting are thorough and accurate. A valid RN license and CPR are required.

Qualifications

• Active RN license with no restrictions.
• CPR certification is current.
• Experience collaborating with a healthcare team.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide skilled nursing care to patients in their homes, including medications, wound care, and assessments.
• Develop and implement individualized care plans with physicians and clinicians.
• Monitor patient conditions and report changes.
• Educate patients and families on disease management, medications, and treatments.
• Maintain accurate records and coordinate with healthcare professionals.
• Report progress to physician and Clinical Manager.
• Oversee Home Health Aides and ensure proper documentation.
