Job summary

Qualifications

Responsibilities

• Educate patients and families on disease management, medications, and treatment options.
• Maintain accurate records of patient care and coordinate with other healthcare professionals.
• Report patient care and progress to physician and Clinical Manager.
• Oversee Home Health Aides and ensure documentation is completed correctly.
