Job summary

CenterWell Home Health is seeking a Home Health Registered Nurse to provide personalized, one-on-one care to patients in their homes. You will work closely with physicians and clinicians to develop and implement care plans that support recovery and independence.

You will administer medications, perform wound care, conduct assessments, and educate patients and families on disease management and treatment options while maintaining thorough documentation and coordinating with the care team.

Qualifications

• Current CPR certification.
• Current and unrestricted RN licensure.
• Experience collaborating with a team of healthcare professionals.

Responsibilities

• Provide skilled nursing care to patients in their homes, including medications, wound care, and assessments.
• Develop and implement individualized care plans with physicians and other professionals.
• Monitor patients' conditions and report changes to physicians and the Clinical Manager.
• Educate patients and families on disease management, medications, and treatment options.
• Maintain accurate records of patient care and coordinate with other healthcare professionals.
• Report patient care progress to the physician and Clinical Manager.
• Oversee Home Health Aides and ensure documentation is completed correctly.
• All duties performed to support patient independence and recovery.
