CenterWell Home Health is seeking a Home Health RN Case Manager to provide admission, case management, and follow-up skilled nursing visits for home health patients.

You will coordinate care with therapists and aides, supervise home health workers, document care plans, and collaborate with physicians under the supervision of the Clinical Manager. RN licensure and driving ability are required.

Qualifications

• Diploma, Associate, or Bachelor Degree in Nursing
• A minimum of one year of nursing experience
• Strong med surg, ICU, ER, acute experience
• Current and unrestricted Registered Nurse licensure
• Current CPR certification
• Strong organizational and communication skills
• A valid driver's license and reliable transportation

Responsabilités

• Provide admission, case management, and follow-up skilled nursing visits for home health patients
• Provide ongoing care and case management for each patient under Clinical Manager supervision
• Confer with physician in developing initial plan of treatment based on orders and assessments
• Revise plan with physician based on ongoing assessments
• Coordinate care with Physical/Therapists, Home Health Aides, and external providers
• Report patient care/condition/progress to physician and Clinical Manager
• Implement care plan with patient and family
• Oversee/supervise Home Health Aides and Personal Care Workers
• Ensure documentation is completed by staff and reflects current plan of care
• Discharge patients after physician and Clinical Manager consultation and finalize records
• Prepare medical documentation including case conferences, patient contacts, and care plan changes
• Prepare visit/shift reports and communicate with other health care disciplines
