Job summary

Safe Life Home Health Care is seeking an office-based Clinical Case Manager to support referral intake, triage, coordination of care, and communication with patients, families, referral sources, and the clinical team. This role is ideal for an RN, LPN, or LVN who thrives in a fast-paced home health setting and handles a high volume of cases.

You'll review referrals, determine eligibility, coordinate with intake and clinical staff, and ensure smooth case flow from referral through admission

Qualifications

• Active RN license in good standing.
• Experience in home health, intake, referral management, or clinical triage.
• Excellent communication, organizational, and multitasking skills.
• Office-based, high-volume case management in a fast-paced setting.
• Familiarity with EMR and payer documentation is helpful.

Responsibilities

• Review and triage incoming home health referrals promptly.
• Coordinate with referral sources, hospitals, physicians, discharge planners, and internal staff to gather required documentation.
• Assess referral eligibility based on clinical needs, payer requirements, service area, and staffing.
• Communicate with patients and families about next steps, services, and admission process.
• Support case assignment and coordination between intake, scheduling, field clinicians, and branches.
• Track referrals and follow up on missing documents, orders, insurance information, and records.
• Prioritize urgent and high-acuity cases for timely response.
• Maintain accurate and timely EMR documentation.
• Resolve referral/admission barriers to improve conversion and patient experience.
• Ensure compliance with company policies and home health regulations.
• Support Lombard and Chicago branch operations as needed.
