Job summary

Safe Life Home Health Care, LLC is seeking an office-based Clinical Case Manager to support referral intake, case triage, coordination of care, and communications with patients, families, referral sources, and the clinical team. This role is ideal for an RN who is organized, responsive, and comfortable managing a high volume of cases in a fast-paced home health setting.

The position serves both Lombard and Chicago branches, focusing on eligibility assessment, care planning, and ensuring smooth

Qualifications

• Active RN license required.
• Experience in home health, intake, referral management, case coordination, or clinical triage preferred.
• Strong understanding of home health services, documentation, and care coordination processes.
• Excellent communication, organizational and multitasking skills.

Responsibilities

• Oversee patient care services and personnel, coordinate patient care and referrals, and maintain updated patient plans of care.
• Act as first point of contact for staff concerns and issues.
• Plan, direct and participate in delivery of home health services with providers and facilities.
• Coordinate with hospitals, physicians, discharge planners, and internal teams for patient care.
• Provide training and conduct employee reviews and coaching.
• Communicate with leadership on quality improvement and case management workflow.
• Collaborate with Nursing Director, Intake, QA, and branch staff to support efficient case management.
