Job summary

Amedisys Home Health is hiring a Registered Nurse Case Manager for full-time day shifts. You will conduct patient assessments, collaborate with the care team to create plans of care, supervise LPNs and HHAs, and ensure timely documentation.

Qualifications

• 1+ year of clinical RN experience.
• Current RN license for assigned state(s).
• Current CPR certification is required.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs.
• Promotes patient health and independence through teaching and rehabilitative measures.
• Supervises LPNs and HHAs.
• Completes timely and accurate documentation.
• Communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities and on-call services as assigned.
• Participates in clinical development and continuing education.
