Job summary

Amedisys is seeking a Registered Nurse Case Manager to provide in-home patient assessments and coordinate a care plan with the interdisciplinary team. You will teach patients self-care techniques and supervise LPNs and HHAs while ensuring timely documentation.

Qualifications

• 1+ year of clinical RN experience; exceptions possible with VP approval.
• Current RN license for assigned state(s).
• Current CPR certification.
• Valid driver’s license, reliable transportation and liability insurance.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Promotes patient health and independence through teaching and rehabilitative measures.
• Supervises LPNs and HHAs; completes documentation timely and accurately.
• Communicates patient progress to the clinical manager and care team.
• Participates in staff education and on-call responsibilities.
