Job summary

Angels Care Home Health in the United States is seeking an experienced RN Case Manager to coordinate all aspects of the patient’s home care in Chanute, Kansas. You will interact with multiple medical disciplines, perform the admission visit, and design the plan of care to meet eligibility for home care services.

The ideal candidate is a graduate of an accredited nursing program, holds an active RN license, a valid driver’s license, and reliable transportation.

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One-year experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient’s home care.
• Perform the admission visit to the patient in the home.
• Develop the plan of care and determine eligibility for home care services.
