Job summary

CenterWell Home Health is seeking a compassionate Home Health Registered Nurse to provide one-on-one skilled nursing care in patients' homes. You will administer medications, perform wound care, and develop care plans with physicians.

Reporting to the Clinical Manager, you will monitor patient progress, educate families, maintain records, coordinate with the care team, and oversee Home Health Aides to ensure proper documentation. A generous $10,000 sign-on bonus is offered.

Qualifications

• Current and unrestricted RN licensure.
• Current CPR certification.
• Experience collaborating with a healthcare team.
• Valid driver's license and reliable transportation.
• Med surg/ICU/ER/acute care experience.

Responsibilities

• Provide skilled nursing care in patients' homes, including medications, wound care, and assessments.
• Develop and implement individualized care plans with physicians and clinicians.
• Monitor patients' conditions and report changes.
• Educate patients and families on disease management and treatment options.
• Maintain accurate records of patient care and coordinate with the care team.
• Report progress to the physician and Clinical Manager.
• Oversee Home Health Aides and ensure proper documentation.
