Suncrest Hospice is seeking a Registered Nurse (RN) to deliver patient care across home and facility settings. The RN will participate in the interdisciplinary team, addressing psychosocial needs and coordinating with community resources.
Travel within the service area is required, with assignments in Case Management, Admissions, or Resource roles. The role emphasizes strong time management, independent work, and the ability to communicate effectively via phone, email, and meetings.

The Registered Nurse (RN) will be responsible for providing care and services for a group of patients, visiting patients where they reside, which could include private homes, Skilled Nursing Facilities, and assisted living facilities. This position is a contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources. The Registered Nurse (RN) will work in either a Case Management, Admissions, or Resource role.
