Job summary

Elara Caring is hiring a Registered Nurse Home Health Case Manager in Akron, OH. This role delivers high-quality, patient-centered home health services, guiding patients along their health journey and coordinating care with the clinical team.

You will report to the Clinical Manager or Clinical Supervisor and travel within the branch coverage area. A nursing degree is preferred, with opportunity for professional growth and supportive benefits.

Qualifications

• Current, unrestricted state RN license
• 1+ year of clinical nursing experience
• Willingness to travel within branch/office coverage area
• Ability to frequently lift up to 50 pounds and reposition/move patients/equipment
• Must have a dependable vehicle, valid driver's license and auto insurance per state laws
• Willingness to travel 50%
• Associates or Bachelor’s Degree in Nursing preferred
• Home Health or Hospice experience preferred
• Experience with HomeCare HomeBase preferred

What is Required?

• Current, unrestricted state RN licensee
• 1+ year experience in a clinical care setting as a nurse
• Able and willing to travel within branch/office coverage area.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
• Must be able and willing to travel 50%
• Associates Degree or Bachelor’s Degree in Nursing is preferred
• Previous Home Health or Hospice Experience is preferred
• Previous experience with HomeCare HomeBase is preferred
