Job summary

Mountaineer Home Care in West Virginia seeks a Registered Nurse to deliver and direct nursing care in patients' homes as prescribed by physicians and in line with regulations and agency policies.

You will coordinate total care with other health professionals, supervise aides, complete assessments, and ensure continuity of care through effective communication with physicians, patients, and families. A solid driver’s license and reliable transportation are required.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other healthcare professionals.
• Supervises care provided by home health aides and LPNs/LVNs, provides instruction, and assigns tasks according to state and federal regulations
• Provides required supervisory visits
• Documents patient visits per policy and payer requirements, and syncs timely per agency policy
• Communicates with physicians, patients, and family to ensure quality care and service excellence
