Job summary

Mountaineer Home Care, part of the LHC Group and within the Optum family, is hiring a Registered Nurse in Home Health to deliver and coordinate nursing care in patients' homes across Kanawha County. Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• Preferred CPR Certification.
• 1+ years of Home Health experience.
• Ability to work independently.
• Solid communication, writing, and organizational skills.

Responsibilities

• Provide and direct nursing care to patients in their homes as prescribed by the physician and in compliance with laws, regulations, and agency policies.
• Coordinate the plan of care with other health care professionals involved in care.
• Perform supervisory visits and oversee home health aides and licensed nurses per state/federal regulations.
• Document visits and care delivery per policy and payer requirements.
• Assess patients (OASIS), reconcile medications, and ensure accurate documentation.
• Communicate with physicians, patients, and families to ensure quality care and service excellence.
• Participate in on-call and weekend rotations as needed.
• Support survey readiness and performance improvement initiatives.
