VitalCaring is seeking a Clinical Care Manager (RN) to lead the coordination of home health care, guiding clinical decisions, and ensuring patients receive high-quality, attentive care.

As a field-based RN, you will manage care across the full patient journey, collaborate with physicians and care teams, and drive outcomes with a patient-centric approach while maintaining compliance and documentation accuracy.

Qualifications

• Active RN license in state of practice.
• Graduate of an accredited nursing program (BSN preferred).
• Home Health experience preferred.
• Experience in care coordination or clinical leadership.

Responsabilités

• Lead and coordinate care across the full patient journey.
• Coordinate and integrate patient care across clinicians, caregivers, and providers.
• Manage physician orders, documentation, and workflows within the EMR.
• Partner with Intake to support timely admissions and reduce referral delays.
• Conduct ongoing patient assessments and ensure plan-of-care alignment.
• Review clinical reports and take appropriate action.
• Lead case conferences, recertifications, and discharge planning.
• Ensure compliance with clinical, state, and federal regulations.
