Job summary

VitalCaring is seeking a Clinical Care Manager (RN) for home health to lead coordination of care across the patient journey. You will guide clinical decisions and ensure high-quality, compliant care delivered in the field and via EMR workflows.

This role emphasizes ownership, collaboration, and the ability to manage complex cases while maintaining strong communication with physicians, patients, and families. A full benefits package is included.

Qualifications

• Active RN license in state of practice.
• Graduate of an accredited nursing program (BSN preferred).
• Home Health experience strongly preferred.
• Experience in care coordination, case management, or clinical leadership preferred.

Responsibilities

• Lead and coordinate care across the full patient journey in home health.
• Manage physician orders, documentation, and workflows in the EMR.
• Partner with Intake to support timely admissions and reduce referral delays.
• Conduct ongoing patient assessments and ensure plan-of-care alignment.
• Lead case conferences, recertifications, and discharge planning.
• Ensure compliance with clinical, state, and federal regulations.
