Job summary

VitalCaring seeks a Clinical Care Manager (RN) to lead coordination of home health, guide clinical decisions, and ensure patients receive quality, attentive care. You’ll act as the clinical quarterback across the patient journey.

In this full-time, field-based role you’ll collaborate with physicians, caregivers, and intake teams, manage orders and documentation in the EMR, conduct assessments, supervise care plans, and ensure compliance with regulations.

Qualifications

• Active RN license in state of practice.
• Graduate of an accredited nursing program (BSN preferred).
• Home Health experience strongly preferred.
• Experience in care coordination, case management, or clinical leadership preferred.
• Strong clinical judgment and ability to manage multiple priorities.
• Confident communicator with the ability to collaborate across disciplines.
• Comfortable working independently in a fast-paced, field-based environment.

Responsibilities

• Coordinate and integrate patient care across clinicians, caregivers, and providers.
• Manage physician orders, documentation, and workflows within the EMR.
• Partner with Intake to support timely admissions and reduce referral delays.
• Conduct ongoing patient assessments and ensure plan-of-care alignment.
• Review clinical reports and take appropriate action.
• Lead case conferences, recertifications, and discharge planning.
• Ensure compliance with clinical, state, and federal regulations.
• Serve as a key liaison between physicians, patients, families, and internal teams.
• Support care delivery through in-home visits as needed.
