Job summary

VitalCaring Group is seeking a Clinical Care Manager (RN) for home health to lead coordination of care, guide clinical decisions, and ensure every patient receives quality, attentive care throughout the full journey.

You will partner with physicians, caregivers, and internal teams, manage orders and EMR workflows, conduct ongoing assessments, lead case conferences and discharge planning, and drive outcomes while maintaining compliance.

Qualifications

• Active RN license in state of practice.
• Graduate of an accredited nursing program (BSN preferred).
• Home Health experience strongly preferred.
• Experience in care coordination, case management, or clinical leadership preferred.
• Strong clinical judgment and ability to manage multiple priorities.
• Confident communicator with the ability to collaborate across disciplines.
• Comfortable working independently in a fast-paced, field-based environment.

Responsibilities

• Lead and coordinate care across the full patient journey.
• Coordinate and integrate patient care across clinicians, caregivers, and providers.
• Manage physician orders, documentation, and workflows within the EMR.
• Partner with Intake to support timely admissions and reduce referral delays.
• Conduct ongoing patient assessments and ensure plan-of-care alignment.
• Review clinical reports (occurrences, infection control, on-call) and take appropriate action.
• Lead case conferences, recertifications, and discharge planning.
• Ensure compliance with clinical, state, and federal regulations.
• Serve as a key liaison between physicians, patients, families, and internal teams.
• Support care delivery through in-home visits as needed.
