Job summary

Horizon Home Health Care in Mercer County, Ohio is seeking a compassionate Registered Nurse (RN) to join our home health team. You will manage patient care from admission through discharge, perform OASIS assessments, and develop individualized plans of care while collaborating with an interdisciplinary team.

This role offers autonomy, a flexible schedule with options for full-time, part-time or PRN, weekly pay, and benefits.

Qualifications

• Current Ohio RN license is required.
• Experience in home health or OASIS assessments preferred.
• Valid driver's license and reliable transportation required.
• Strong assessment, communication, organizational and time management skills.

Responsibilities

• Performs SOC, ROC, Recertification, Transfer, and Discharge OASIS assessments.
• Conducts comprehensive nursing assessments and develops individualized plans of care.
• Coordinates and manages patient care throughout the episode of care.
• Provides skilled nursing services, patient education, medication management, and disease management.
• Collaborates with physicians, therapists, social workers and other members of the interdisciplinary team.
• Monitors patient progress and adjusts care plans as needed.
• Completes accurate OASIS documentation and clinical records per Medicare/state/agency guidelines.
• Participates in quality improvement initiatives and regulatory compliance.
