Job summary

Horizon Home Health Care in Montgomery County, OH seeks a compassionate Registered Nurse to manage patient care from admission through discharge. You will perform SOC/OASIS assessments, develop individualized care plans, educate patients, and coordinate with physicians, therapists and social workers to achieve optimal outcomes.

This role requires current Ohio RN license and knowledge of Medicare home health documentation; CPR/BLS certification and valid driver's license are preferred.

Qualifications

• Current Ohio RN license.
• Experience in home health and OASIS assessments preferred, but not required.
• Knowledge of Medicare home health regulations and documentation requirements preferred, but not required.
• Current CPR and/or BLS certification.
• Valid driver's license, reliable transportation, and proof of auto insurance.
• Strong assessment, communication, organizational and time management skills.

Responsibilities

• Performs Start of Care (SOC), Resumptions of Care (ROC), Recertification, Transfer, and Discharge OASIS assessments.
• Conducts comprehensive nursing assessments and develop individualized plans of care.
• Coordinates and manages patient care throughout the episode of care.
• Provides skilled nursing services, patient education, medication management, and disease management.
• Collaborates with physicians, therapists, social workers and other members of the interdisciplinary team.
• Monitors patient progress and adjust care plans as needed.
• Completes accurate and timely OASIS documentation and clinical records in accordance with Medicare, state and agency guidelines.
• Participates in quality improvements initiatives and maintains compliance with regulatory standards.
