You will assess, plan and implement patient care in patients' homes, educate families on disease processes, medications, and procedures, and address diverse patient needs including IVs, PICC lines, wound care, trachs, G/J tubes and Foley catheters. Qualifications

• One year of acute care experience required.
• CPR certification required.
• Ability to travel for in-home patient visits.
• Exceptional time-management skills.
• Excellent interpersonal and customer service skills.
• Home healthcare/case management experience preferred.

Responsibilities

• Assess, plan and implement patient care in the home setting.
• Educate patients and families on diseases, procedures and medications.
• Address patient needs including IVs, PICC lines, wound care, trachs, G/J tubes and Foley catheters.
• Monitor and ensure medication adherence and administration in the home.
• Provide post-operative care and monitor vitals, chest pain, blood pressure and edema.
• Perform dressing changes and wound care; manage wound vacs as needed.
• Support continuity of care and build long-term patient relationships.
• Utilize case management processes and work with the care team to coordinate services.
