Job summary

Horizon Home Health Care in Miami County, Ohio seeks a compassionate Registered Nurse (RN) to manage patient care from admission to discharge in home health settings. The RN will perform SOC/ROC/recertification, develop individualized care plans, and collaborate with an interdisciplinary team to achieve optimal outcomes while maintaining compliance and high-quality documentation.

Qualifications

• Must hold an Ohio RN license.
• Experience in home health and OASIS assessments preferred, but not required.
• CPR and/or BLS certification.
• Valid driver's license with reliable transportation.
• Strong assessment, communication, organizational and time management skills.

Responsibilities

• Performs SOC, ROC, Recertification, Transfer and Discharge OASIS assessments.
• Conducts comprehensive nursing assessments and develops individualized plans of care.
• Coordinates and manages patient care throughout the episode of care.
• Provides skilled nursing services, patient education, medication management, and disease management.
• Collaborates with physicians, therapists, social workers and other team members.
• Monitors progress and adjusts care plans as needed.
• Completes accurate OASIS documentation and records per regulations and guidance.
• Participates in quality improvements and ensures regulatory compliance.
