Qualifications

• Active RN license in the state you work.
• At least 1 year of RN experience delivering skilled care.
• Valid driver’s license and ability to drive to patient locations.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting in collaboration with an interdisciplinary team.
• Perform patient assessments, medication management, and care planning.
• Educate and empower patients and families on disease management and treatment plans.
