Job summary

Residential Home Health and Residential Hospice in Harrisburg, IL is hiring a Home Health RN to deliver skilled nursing care to patients at home. Qualifications

• Active RN license in the state you work.
• One year of RN experience and clinical expertise.
• Valid driver’s license and ability to spend ~20% of workday driving to patient locations

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform patient assessments, medication management, and care planning
• Educate and empower patients and families on disease management and treatment plans so they feel confident, informed, and cared for
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory
