Residential Home Health and Hospice in Illinois is seeking a dedicated Registered Nurse (RN) to provide skilled nursing care in patients' homes in Northfield and surrounding areas. This role offers autonomy, meaningful patient relationships, and real career growth, with a $10,000 first-year success bonus and ongoing rewards.

You will assess patients, manage medications, develop care plans, educate families on disease management, document in EMR, and travel within your territory.

Qualifications

• Active RN license in the state you work
• Minimum 1 year of RN experience
• Valid driver’s license and ability to travel ~20% of workday
• Ability to work independently within an interdisciplinary team

Responsabilités

• Deliver skilled, patient-centered care in a home health setting
• Perform patient assessments, medication management, and care planning
• Educate and empower patients and families on disease management and treatment plans
• Think critically and ensure thorough EMR documentation
• Work independently, but collaborate with the interdisciplinary team
• Travel to patient homes within assigned territory
