Job summary

Residential Home Health and Hospice in Mount Vernon, IL is hiring a registered nurse (RN) for a Home Health position. You will perform patient assessments, manage medications, educate patients and families, and document accurately in the EMR. Travel to patient homes within your territory is required.

Qualifications

• Active RN license in the state you work
• Minimum 1 year RN experience delivering skilled home health care
• Valid driver's license and ability to travel ~20% of the day

Responsibilities

• Deliver skilled, patient-centered care in home health settings with an interdisciplinary team
• Perform patient assessments, medication management, and care planning
• Educate and empower patients and families on disease management and treatment plans
• Think critically, solve problems in real time, and document in EMR
• Work independently, but collaborate with the team and travel to patient homes
