Intermountain Health in Denver, CO is seeking a compassionate Registered Nurse to provide in-home patient-focused care. You will assess, plan, implement, and evaluate nursing care tailored to each patient’s goals, while coordinating with a multidisciplinary team.

Experience in home health, strong communication, and BSN are preferred. A valid RN license, BLS, and a driver's license are required. This position offers flexible PRN scheduling and mileage reimbursement.

Qualifikationen

• Current RN license in the state of practice.
• Basic Life Support (BLS) certification for healthcare providers.
• Valid driver’s license with reliable transportation and current auto insurance.
• Bachelor’s Degree in Nursing (BSN) is required as of last update 4/27/2025.
• Experience: RNs with less than 12 months must complete BSN within five years of start date.

Aufgaben

• Assess, plan, implement, and evaluate home-based patient care.
• Coordinate with multidisciplinary team to support patient independence.
• Provide wound care, medication/injection administration, IV therapy monitoring, and patient/caregiver education.
• Document care accurately and maintain charting.
• Communicate clearly with patients, families, and healthcare teams.
