Job summary

VitalCaring Group is seeking a Field-based Registered Nurse (RN) to lead in-home patient care and manage the plan of care from admission through discharge. You’ll apply advanced assessment skills, coordinate with physicians and care teams, and educate families to support independence at home.

This role emphasizes clinical autonomy, high-impact care, and strong time management in a flexible schedule with robust support from a collaborative clinical team. A valid RN license and CPR are required.

Qualifications

• Active RN license and CPR certification.
• Graduate of an accredited nursing program.
• Minimum one year RN experience in Home Health or related areas.
• Strong clinical judgment and ability to think independently.
• Excellent communication skills and patient-centered approach.
• Ability to manage a caseload and prioritize effectively.
• Proficiency with EMR systems.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct comprehensive in-home assessments to evaluate clinical status, risks, and goals of care.
• Develop and manage individualized care plans in collaboration with physicians and care teams.
• Deliver skilled nursing care, including medication management, treatments, and interventions.
• Serve as the case manager coordinating care across therapists, aides, and physicians.
• Monitor patient progress and make clinical decisions based on changing conditions.
• Educate patients and families to support disease management and independence.
• Coordinate care transitions and discharge planning to reduce readmissions.
• Maintain timely, accurate documentation in the EMR.
• Supervise and support Home Health Aides as needed.
• Participate in on-call rotation as appropriate.
