VitalCaring Group is seeking a Home Health Registered Nurse to lead patient care in home settings within an assigned territory. You will assess, plan, and deliver skilled nursing care, coordinating with physicians and interdisciplinary teams to optimize outcomes and independence at home.

This role emphasizes clinical autonomy, complex and high-impact care, and a strong focus on patient relationships. Flexible scheduling supports work-life balance while maintaining high-quality care.

Qualifications

• Active RN license (or compact eligibility) + CPR certification.
• Minimum one year RN experience (Home Health, Med-Surg, ICU, or Acute Care preferred).
• Graduate of an accredited nursing program.

Responsabilités

• Conduct comprehensive in-home assessments to evaluate clinical status, risks, and goals of care.
• Develop and manage individualized care plans in collaboration with physicians and care teams.
• Deliver skilled nursing care, including medication management, treatments, and interventions.
• Serve as the case manager, coordinating care across therapists, aides, and physicians.
• Monitor patient progress and make clinical decisions based on changing conditions.
• Educate patients and families to support disease management and independence.
• Coordinate care transitions and discharge planning to reduce readmissions.
• Maintain timely, accurate documentation in the EMR.
• Supervise and support Home Health Aides as needed.
• Participate in on-call rotation as appropriate.
