Job summary

Pathways Healthcare is seeking a Registered Nurse - Home Health to serve Methuen and surrounding areas. This full-time role operates Monday through Friday, 8:30 AM to 5:00 PM, focusing on patient-centered home-based care and AI-assisted documentation to enhance clinician productivity.

The ideal candidate will have 2+ years of nursing experience (geriatrics preferred), a nursing degree, and proficiency with EMR systems, with strong communication and knowledge of healthcare regulations.

Qualifications

2+ years nursing experience, preferably in geriatrics is desirable.
Degree in Nursing from an accredited institution.
Proficient with electronic medical records.
Excellent bedside manner and communication skills.
Working knowledge of health care laws and regulations.
Maintain current BLS CPR certification.
Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

Assesses and evaluates patient’s status by: writing and initiating plan of care.
Regularly re-evaluating patient and family/caregiver needs.
Participating in revising the plan of care as necessary.
Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions.
Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given.
Records pain/symptom management changes/outcomes as appropriate.
Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
Provides and maintains a safe environment for the patient.
Attends interdisciplinary group meetings.
