Pathways Healthcare in Massachusetts is seeking an experienced registered nurse for our home-based Medical Rehabilitation Program. You will perform preventive and rehabilitative nursing procedures, assess patients, administer medications, and develop care plans in collaboration with physicians and the care team.

You must hold an active RN license in the state, have 2+ years of geriatrics experience, a nursing degree, and current BLS CPR.

Formación

• Nursing degree from an accredited institution.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Valid driver's license and reliable vehicle for visits.

Responsabilidades

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial patient assessments and determine home care needs.
• Perform thorough physical assessments and gather medical histories.
• Develop and revise individualized plans of care.
• Collaborate with physicians and interdisciplinary teams to plan care.
• Address inquiries from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers.
• Supervise home health aides as required.
• Identify discharge planning needs and implement them.

Job Description:

• Role: Registered Nurse
• Main Office Location: Concord, MA
• Service Area: L owell area
• Schedule: Flexible

Qualifications:

• Must hold an active RN license in the state where the position is based.
• Preferably 2+ years of nursing experience, with a focus on geriatrics.
• Nursing degree from an accredited institution.

• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

• Initiate appropriate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status and evolving needs.
• Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
• Responsively address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them as part of the care plan development.
