Job summary

Optum's VNA of Maryland, part of LHC Group, is hiring an RN for home health in Prince George's County. You will direct nursing care at patients’ homes, coordinate with physicians and the care team, and support continuity of care in a compliant and compassionate setting.

The role requires active RN licensure in the state, valid driver's license, and CPR certification (preferred). A $7,500 sign-on bonus is available for external candidates.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License with access to a dependable vehicle or transportation.
• CPR Certification preferred.

Responsibilities

• Initiates and revises the plan of care in collaboration with the physician and health professionals.
• Supervises home health aides and licensed nurses; provides instruction and assigns tasks per regulations.
• Provides supervisory visits and ensures quality clinical services within plan of care.
• Completes comprehensive nursing assessments (OASIS) and medication reconciliation timely.
