Optum's VNA of Maryland home health team seeks a Registered Nurse to provide and direct nursing care to patients in their homes as prescribed by the physician, coordinating with the care team to maintain continuity of care.

Responsibilities include developing and revising the plan of care, supervising aides, completing OASIS assessments, and communicating with physicians, patients, and families to ensure high-quality care. A $7,500 sign-on bonus is available for external candidates.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• Preferred CPR Certification or ability to complete within 90 days of hire.
• 1+ years of Home Health experience.
• Ability to work independently.
• Solid communication, writing, and organizational skills.

Responsabilités

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed nurses, provides instruction, and assigns tasks according to State and federal regulations.
• Provides required supervisory visits and completes comprehensive assessments (OASIS).
• Documents patient visits per policy and payer requirements, and ensures timely communication with the care team.
• Communicates with physicians, patients, and families to ensure quality care and service excellence.
