VNA of Maryland, a part of LHC Group and part of the Optum family, is seeking an Occupational Therapist in Home Health to assess functional status, daily living activities, and home safety needs for patients.

You will evaluate therapy plans with the care team, document CMS requirements, and implement evidence-based interventions in patients' homes. A $10,000 sign-on bonus is available for external candidates.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and access to dependable vehicle or public transportation.
• Ability to function in any home situation regardless of client demographics.

Responsabilités

• Provides service within the scope of practice and coordinates with the health care team.
• Evaluates functional status and CMS documentation; consults with physician on therapy plan of care.
• Observes and reports patient responses to treatment and condition changes.
• Conducts patient assessments of functional level using relevant tests.
