Job summary

UnitedHealth Group is seeking a Registered Nurse in Home Health for Prince George's County, MD. Provide and direct nursing care in patients' homes as prescribed by physicians and ensure continuity of care with the care team.

Responsibilities include developing and revising plans of care, supervising home health aides, documenting visits and assessments, and coordinating with physicians and family. A $7,500 sign-on bonus is available for external candidates.

Qualifications

• RN licensure in state of practice
• Valid driver's license, vehicle insurance, access to reliable transportation
• CPR certification or ability to obtain within 90 days (preferred)

Responsibilities

• Provides and directs nursing care to home-based patients as prescribed by physician
• Coordinates plan of care with health care professionals and family
• Performs supervisory visits and ensures compliance with regulations and policies
• Documents visits and OASIS assessments accurately and timely
• Communicates patient status and care plan with physicians and care team
