Responsibilities

• Deliver skilled, patient-centered care in patients' homes with an interdisciplinary team
• Perform patient assessments, medication management, and care planning
• Educate patients and families on disease management and treatment plans
• Ensure compliant EMR documentation
• Work independently but with team support
• Travel to patient homes within assigned territory
