Residential Home Health and Residential Hospice is seeking a Registered Nurse (RN) for a Home Health position in Bensenville, IL. This role offers 40 hours per week, an employee type, and a $15,000 bonus, with travel to patient homes within the assigned territory.

The RN will deliver skilled nursing care in patients' homes, perform assessments, manage medications, educate families, and maintain compliant EMR documentation in a supportive interdisciplinary team.

Qualifications

• Active RN license in the state you work.
• One year of RN experience.
• Valid driver’s license and ability to spend ~20% of time driving to patient locations.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting.
• Perform patient assessments, medication management, and care planning.
• Educate patients and families on disease management and treatment plans.
• Think critically, document thoroughly in EMR.
