Qualifications

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create home health care plans aligned with orders and patient goals.
• Administer medications, treatments, and interventions; provide pain and symptom management.
• Educate and support patients' families and caregivers.
• Collaborate with an interdisciplinary team and maintain documentation.
• Participate in on-call rotation as required by the local branch.
