Adoration Health is seeking a dedicated Home Health RN in Abingdon, VA. The role offers full-time hours, a patient-centered approach, and the opportunity to work within a supportive team while contributing to high-quality home healthcare.

You will provide direct nursing care, develop individualized care plans, educate families, and collaborate with the care team to ensure optimal outcomes for patients in their homes.

Qualifications

• Registered Nursing Degree (Associate or Bachelor) with current unrestricted state license.
• One year nursing practice in a patient care setting; home health/geriatrics preferred.
• Valid driver's license with acceptable driving record and insurance as per policy.
• New nursing graduates may be considered in select markets.
• Current CPR certification.

Responsabilités

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create home health care plans aligned with MD orders and patient goals.
• Direct nursing care including medications, treatments, and symptom management.
• Educate and support patient’s family and caregivers.
• Collaborate with an interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by the local branch.
