Job summary

Adoration Health is seeking a passionate Home Health Registered Nurse to join our team in Belpre, OH. This RN will provide patient-centered care in a home-health setting, delivering 1:1 care with a focus on independence and family education.

Responsibilities include creating care plans, administering treatments, and coordinating with a multidisciplinary team. Eligible candidates will hold an RN license and CPR certification, with preferred home health or geriatrics experience.

Qualifications

• RN license in state of practice and residence is required.
• One year nursing practice in patient care setting required; home health/geriatrics preferred.
• Current CPR certification.

Responsibilities

• Assess, monitor physical, emotional, and psychological needs of patients.
• Create home health care plans aligned with MD orders and patient goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patient’s family and caregivers.
• Collaborate with interdisciplinary team and maintain timely documentation.
• Participate in on-call rotation as required by the local branch.
