Job summary

Elara Caring is seeking a Registered Nurse Home Health to provide compassionate, patient‑centered care in patients’ homes across the branch area. The role emphasizes autonomy, 1:1 patient interaction, and ongoing collaboration with a multidisciplinary team.

Applicants should hold an active RN license in Ohio with at least 1 year of clinical nursing experience and be willing to travel up to 50% while maintaining high standards of care and HIPAA compliance. A nursing degree (ADN/BSN) is preferred.

Qualifications

• Current, unrestricted state RN license.
• 1+ year of clinical care experience as a nurse.
• Willing to travel within branch/office coverage area.
• Capable of lifting, pushing, pulling, and transferring up to 50 pounds.
• Valid driver’s license, dependable vehicle, and current auto insurance.
• Willing to travel up to 50%.
• Associates or Bachelor’s Degree in Nursing preferred.
• Home Health or Hospice experience preferred.
• HomeCare HomeBase experience preferred.

Responsibilities

• Ensure activities align with Elara Caring’s board, executive team, and Home Health leadership.
• Maintain patient and staff privacy and confidentiality under HIPAA rules.
• Assess caseload of home health patients and families to identify needs from charts, reports, and on-site evaluations.
• Assume primary responsibility for caseload, including planning, coordinating, implementing, and evaluating the plan of treatment.
• Assess patient needs and gather data on physical, psychological, social, and spiritual factors to inform the care plan.
• Make initial evaluations and follow-up visits; conduct re-evaluations as needed.
• Communicate significant findings to the Clinical Supervisor, physician, facility, and other involved personnel.
• Revise the plan of care in response to issues and notify the team.
