Qualifications

• RN degree from an accredited college of nursing with current unrestricted registration and license in the state of practice and residence.
• One year nursing practice in patient care settings preferred; home health/geriatrics a plus.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create home health care plans aligned with MD orders and patient goals.
• Direct nursing care: administer medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with an interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by the local branch.
