Job summary

Adoration Health in Belpre, OH is seeking a dedicated Home Health Physical Therapist to deliver patient-centered care in the home setting. You will evaluate and treat patients based on physician orders and work 1:1 with families to improve independence and function.

Qualifications

• CAPTE-approved PT education required.
• Current PT licensure in the state and CPR certification required.
• Two years of adult PT experience; home health preferred.

Responsibilities

• Perform PT evaluation and treatment per physician orders, including adaptive equipment procurement.
• Establish goals and develop treatment plans to maximize independence and function.
• Maintain communication among the treatment team, physician, patient and family.
• Document services timely for accurate billing; supervise PTAs and PT Aides as applicable.

Skills

Two years of experience in adult PT

Education

CAPTE-approved PT education

State licensure as Physical Therapist

CPR certification (AHA)
