Job summary

Residential Home Health and Residential Hospice in Troy (MI) is seeking an experienced RN to provide skilled home health care in patients' homes. This role offers autonomy, meaningful patient relationships, and opportunities for growth.

The RN will perform assessments, medication management, and care planning while collaborating with an interdisciplinary team. A $10,000 first-year bonus rewards achievement during the initial year, with ongoing rewards as you grow with the company.

Qualifications

• Active RN license in the state you work
• One year of RN experience
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team
• Perform patient assessments, medication management, and care planning
• Educate and empower patients and families on disease management and treatment plans
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
