Residential Home Health and Hospice in Florida is seeking an RN to provide skilled nursing care in patients' homes in Harbor Beach and surrounding areas. You’ll deliver autonomous, patient-centered care with support from an interdisciplinary team, with a $10,000 first-year success bonus available.

This role emphasizes comprehensive assessments, medication management, care planning, and education for patients and families.

Formación

• Active RN license in the state you work.
• At least 1 year of RN experience in home health or similar clinical setting.
• Valid driver's license and ability to drive between patient homes.

Responsabilidades

• Deliver skilled, patient-centered care in a home health setting with team support.
• Perform patient assessments, medication management, and care planning.
• Educate patients and families on disease management and treatment plans.
• Think critically, document thoroughly in EMR, and ensure compliant records.
• Work independently while staying connected with the interdisciplinary team.
• Travel to patient homes within assigned territory.

RN Qualifications

• Active Registered Nurse, RN license in the state you work
• One year of RN experience and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
