Residential Home Health and Hospice is hiring a Registered Nurse (RN) to provide skilled nursing care to patients in their homes across Tecumseh, Adrian, Britton, and surrounding areas. This role offers autonomy, meaningful patient relationships, and real career growth within a nationally recognized home health provider.

You will perform patient assessments, medication management, and care planning; educate families on disease management; and ensure compliant EMR documentation.

What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform patient assessments, medication management, and care planning
• Educate and empower patients and families on disease management and treatment plans so they feel confident, informed, and cared for
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation

• Work independently, but never alone
• Travel to patient homes within assigned territory

RN Qualifications

• Active Registered Nurse, RN license in the state you work
• One year of RN experience and the clinical expertise to deliver quality patient care

• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
