Puget Sound Home Health is hiring a Licensed Occupational Therapist to provide comprehensive in-home care across Thurston County. You will perform functional assessments and develop client-centered OT plans tailored to improving ADLs, IADLs, safety, and environmental adaptations.

The role emphasizes collaboration with interdisciplinary teams, thorough documentation, and ongoing professional development in home health and aging-in-place strategies.

Qualificações

• Licensed OT in Washington state or eligible
• 1–2 years OT experience
• Current BLS certification

Responsabilidades

• Provide comprehensive patient care with functional assessments and client-centered treatment plans in home health settings.
• Develop and implement individualized OT care plans focusing on ADLs, IADLs, safety, participation, and environmental adaptations.
• Educate patients and families on therapeutic techniques, compensatory strategies, energy conservation, home exercise programs, and safe use of adaptive equipment.
• Collaborate with interdisciplinary teams to ensure continuity of care and safe discharge planning.
• Maintain accurate documentation of progress, outcomes, and reasoning per OT standards.

• Develop and implement individualized OT care plans focused on improving ADLs, IADLs, safety, participation, and environmental adaptations.
• Educate patients and families on therapeutic techniques , compensatory strategies, energy conservation, home exercise/engagement programs, and safe use of adaptive equipment.
• Collaborate with interdisciplinary teams to ensure continuity of care , effective communication, and safe discharge planning.
• Maintain accurate documentation of patient progress, functional outcomes, and clinical reasoning while adhering to professional OT standards and medical terminology.
• Stay current with best practices in occupational therapy , particularly in home health, geriatrics, functional rehabilitation, and aging-in-place strategies.

Qualifications

• Licensed Occupational Therapist in Washington state (or before your first day of work)
• One to two years of experience as a Occupational Therapist
• Current BLS certification
