Job summary

Inspired Home Care is seeking a compassionate Home Health Nurse to manage a patient caseload in-home, delivering clinical visits, wound care, Oasis assessments and IV therapy while educating families. This PRN/Part-Time role requires strong independence and time management as you coordinate with therapists and physicians to meet patient needs.

Travel between patients is expected; a State RN license is mandatory and prior in-home care experience is preferred.

Qualifications

• State RN license required.
• 1 year of in-home care experience preferred.
• Excellent communication and time management skills.

Responsibilities

• Assess patients' condition during visits and chart observations.
• Perform vital signs, medication review, Oasis assessments, wound care and IV therapy.
• Dress wounds and monitor healing progress.
• Educate patients and families on home health care strategies.
• Coordinate with therapists and physicians involved in the care plan.
• Recommend devices or tools to improve quality of life.
• Listen to patient concerns and respond accordingly.

Inspired Home Care is a 5-Star Agency ACHC accredited rapidly expanding and we are hiring PRN and Part Time effective immediately. You will be responsible for managing your own caseload, so time management skills and the ability to work independently are essential. However, it is important to provide each patient with the time and attention necessary to meet his or her needs. Home Health Nurse Responsibilities:

• Assess patients' condition during every visit and chart your observations
• Perform evaluation tasks including vital signs and medication review, Oasis assessment, wound care and IV therapy
• Dress or re-dress open wounds and assess the progress of healing
• Educate patients and their families on proper home health care strategies and procedures
• Coordinate with Occupational Therapists, Physical Therapists, Speech Therapists, Physicians and anyone else involved in the patient's care plan
• Make recommendations for devices or tools that might improve the patient's quality of life
• Listen to the patient and respond to concerns or requests

Qualifications:

• State RN license required
• 1 year of experience in in-home care
• Excellent communication and time management skills

• Ability to travel to multiple patients per day
